Leg Length Discrepancy and Back Pain: How Chiropractic Care Can Help

September 29, 2026

Do you notice one pant leg dragging more than the other, or a sore back after hours on your feet? You could be experiencing a leg length discrepancy (LLD), a surprisingly common difference in the length of your legs. Small differences are perfectly normal, but when one leg is measurably shorter, the pelvis can tip toward that side and the spine bends to compensate. Over time, that uneven foundation can overwork the joints, discs, and muscles of the low back.

WHY ACCURATE LEG LENGTH MEASUREMENT MATTERS

Research shows that measuring a leg length difference takes care and consistency. Tipton, Sutherland, and Schwarzkopf (1) reported that estimates from a routine pelvic X-ray differed significantly from true leg length, and recommended assessing any preexisting discrepancy before hip replacement surgery. Kadhim, Kelly, and Sabharwal (2) determined that three full-length standing X-ray methods yielded similar results, but advised using one technique consistently, particularly when the legs bow or angle. The source of the difference matters as well. Tolk, Eastwood, and Hashemi-Nejad (3) linked leg length differences in Perthes' disease to the top portion of the thigh bone near the hip. Birch, Makarov, and Jo (4) showed that when one part of a child's leg is shorter, the healthy part of that same leg generally doesn't grow longer to make up the difference. You can trust your chiropractor to figure it all out for you!

HOW CHIROPRACTORS ADDRESS LEG LENGTH AND BACK PAIN

This is where your chiropractor can make a real difference. Chiropractic's most important contribution is noticing the discrepancy and recognizing how it affects spinal mechanics in ways that can cause pain. Your assessment may include reviewing pelvic level, posture, and gait, along with X-rays if necessary. Your care plan may then include precise spinal adjustments, flexion-distraction to ease pressure on sensitive joints and discs, targeted exercises, and heel lifts when appropriate. If a discrepancy needs orthopedic care, your chiropractic physician can certainly arrange that referral.
